Appointments and Office Hours
For your convenience we see patients at our Denville, Flanders and Mine Hill locations and offer evening hours in Denville office on Monday, Tuesday and Wednesday evenings. Please call our office at 973.627.0060 to schedule an appointment at any of our locations.
Initial Visit
On your initial visit, please bring the following:
- Your insurance card
- Photo identification
- Your referral, if required by your insurance company
- List of allergies
- List of current medications (prescription or over-the-counter)including:
- Name
- Dosage
- Frequency
- Related X-rays
- Records from the referring physician
- You may also complete the new patient paperwork in advance.
Also, please be prepared to share your medical history with our physicians, including any medical conditions you have or may have had in the past.

Insurance Information
While we accept Medicare and most other insurance plans, it is important to check your insurance provider's list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance customer service representative the rules and limitations on coverage and reimbursement.
